What’s a herniated disk?
A herniated disk (also called a slipped disk, ruptured disk, or bulging disk) occurs when the soft, jelly-like center of a spinal disk pushes through a crack in its tougher outer ring. According to Cleveland Clinic, more than 3 million people in the U.S. get a herniated disk every year, and it’s one of the most common causes of back, neck, and leg pain. The condition most often affects the lower back (lumbar spine) but can also occur in the neck (cervical spine). Research published in the National Library of Medicine shows that 90% of symptomatic herniated disks resolve within 6 weeks with conservative treatment.
What You Might Be Experiencing
But here’s what those statistics don’t capture: the sharp, shooting pain that travels down your leg when you bend over to tie your shoes. The constant ache in your lower back that makes sitting at your desk unbearable. The sleepless nights when you can’t find a comfortable position. The frustration of being told “just rest and take ibuprofen” when you’ve been doing that for weeks and nothing’s changing.
We see patients with herniated and bulging disks who’ve been offered two options: physical therapy that didn’t work, or surgery they’re trying desperately to avoid. Sometimes they’ve been in pain for months—or even years—being told “let’s just give it more time” while their quality of life gets worse. If this sounds familiar, you’re not alone. And there’s another option.
How We See It Differently
Here’s what we’ve learned: a herniated disk is rarely happening in isolation. Usually there’s muscle tightness pulling on your spine (like the piriformis or psoas muscles), joint dysfunction making your back compensate in unhealthy ways, and inflammation keeping the whole cycle going. That’s why a one-size-fits-all approach rarely works. We use flexion distraction therapy—a specialized table technique that gently stretches the vertebrae, increases space for the disk, and actually helps pull herniated material back into place. It’s a proven alternative to surgery that most people have never even heard of.
Below, we’ll walk through what causes herniated and bulging disks, how we treat them at our Elkhorn practice, and what you can realistically expect from treatment. But if you’d rather just talk to someone who gets it, you can book a free 15-minute consultation or contact us directly.
What Causes Herniated and Bulging Disks?
A herniated disk doesn’t usually happen overnight. It’s the result of multiple factors building up over time. And in our experience, there’s almost always more than one thing going on.
Improper lifting or sudden movements
Lifting heavy objects (kids, grandkids, gym weights, boxes) with poor form puts enormous stress on your spinal disks. Sometimes a single awkward movement is enough to push a disk past its breaking point, especially if it’s already weakened from years of wear.
Prolonged sitting and poor posture
Sitting for hours every day—at a desk, in the car, on the couch—puts constant pressure on your lower back disks. Add in slouching or leaning forward, and that pressure increases even more. Your body wasn’t designed to stay in one position for 8+ hours a day.
Muscle imbalances and tightness
When muscles like the piriformis (deep glute muscle), psoas (hip flexor), or lower back muscles get tight or weak, your spine has to compensate. This puts uneven pressure on your disks and can cause or worsen a herniation. We see this constantly in patients who sit all day or have old injuries they never fully addressed.
Natural wear and tear (degenerative disk disease)
As we age, spinal disks naturally lose water content and become less flexible. This makes them more vulnerable to tears and herniations. According to research, approximately 90% of degenerative disk disease occurs in the lowest portions of the lumbar spine.
Previous injuries
Old car accidents, sports injuries, or falls can leave lingering weakness in your spine—even if you thought you recovered. Years later, that weakened area becomes the site of a herniation.
Excess weight and inflammation
Extra body weight puts additional pressure on your lower back disks. Chronic inflammation—often driven by diet, stress, or underlying health issues—can also make disks more prone to injury and slower to heal.
Most people have more than one of these issues at play. That’s why a one-size-fits-all approach rarely works—and why we look at the whole picture before recommending treatment.
How We Treat Herniated and Bulging Disks
Once we understand what’s causing your disk herniation, here are the treatments that might help:
Flexion Distraction Therapy
Our specialized treatment table gently stretches your lower back in ways you can’t do on your own. This increases the space between vertebrae, allows more nutrients into the disk, and can actually help pull herniated material back into place. It’s one of the most effective conservative treatments for disk herniations and a proven alternative to surgery.
Chiropractic Adjustments
When your spinal joints aren’t moving properly, your muscles compensate and disks take extra pressure. We use techniques like drop table (Thompson) or side-posture (Gonstead) adjustments to restore proper movement and take stress off the affected disk.
Deep Muscle Work (Piriformis & Psoas Release)
The piriformis and psoas muscles can pull on your spine and compress disks when they’re tight. We release these deep muscles to take pressure off your lower back and give the disk space to heal. This work isn’t always comfortable, but patients tell us it makes a huge difference.
Shockwave Therapy
For chronic pain and inflammation around the herniated disk, we use pressure wave therapy (also called shockwave) to increase blood flow, break up scar tissue, and speed healing in injured tissue.
Functional Medicine Testing
If inflammation is part of what’s keeping your disk from healing, we can run comprehensive tests to find the root cause—food sensitivities, nutrient deficiencies, hormonal imbalances—and address it from the inside out.
Where you start depends on what makes sense for you right now. Some patients come in specifically for flexion distraction, others discover it as part of their broader care plan.
What to Expect from Treatment
Your First Appointment (60 minutes)
We’ll go through your full health history, talk about when the pain started and what makes it better or worse, and do a physical exam to see what’s happening with your spine and muscles. From there, we’ll figure out which treatments make the most sense and create a plan.
The First Few Weeks
Most patients start with 2-3 visits per week for the first couple of weeks. This gives us enough frequency to make real progress without overwhelming your schedule. You’ll likely notice some relief after just a few sessions—many people feel better even after the first visit—but real healing takes time.
Timeline for Improvement
According to medical research, 90% of symptomatic herniated disks improve with conservative treatment within 6 weeks. In our experience, most patients see meaningful changes within 4-6 weeks when they follow the treatment plan consistently. Some improve faster, some take a bit longer—it depends on how long you’ve had the problem and what else is going on.
What Happens During Flexion Distraction
You’ll lie face-down on a special table that gently moves your lower back up and down in a slow, rhythmic motion. It stretches the lumbar spine in ways your body can’t do on its own and creates space for the disk to heal. Almost everyone says it feels good—like a relief they didn’t know they needed.
Long-Term Maintenance
Once your pain is under control, we’ll work on strengthening and stabilizing so it doesn’t come back. Some patients continue with occasional maintenance visits, others feel good and move on. We meet you where you’re at.

Frequently Asked Questions
Is flexion distraction safe for herniated disks?
Yes. Research shows that spinal traction and distraction therapy are effective, evidence-based treatments for lumbar disk herniation. The technique is gentle, controlled, and specifically designed to help herniated material move back into place while reducing pressure on nerves.
Do I need an MRI before starting treatment?
Not always. We can often tell what’s going on through your history and physical exam. If you’re not improving after a few weeks of treatment, or if we suspect something more serious, we’ll refer you for imaging.
How is this different from regular physical therapy?
Flexion distraction uses a specialized table that stretches your spine in ways that manual therapy can’t replicate. It’s particularly effective for disk issues because it creates space between vertebrae and helps pull herniated material back into position. We also address muscle tightness, inflammation, and other underlying causes—not just the symptoms.
Will I need surgery?
Most people don’t. According to research, 90% of sciatica cases due to lumbar disk herniation resolve with conservative treatment. Surgery provides faster relief in some cases, but long-term outcomes after 1 year are similar to conservative management. We’ll monitor your progress closely and refer you to a surgeon if needed—but the goal is to help you avoid that.
Do you take insurance?
We’re an out-of-network provider, which means we’re not limited by insurance restrictions on testing or treatment length. Many patients save money compared to conventional care once you factor in copays, deductibles, and out-of-pocket costs. We provide superbills you can submit to your insurance for potential reimbursement.
How much does treatment cost?
It varies depending on what you need. We’ll discuss cost during your consultation or first appointment so there are no surprises.
