Tired, gaining weight, and told it’s just age?

Dr. Julie Gurbacki helps men in Omaha and Elkhorn get to the bottom of low energy, stubborn weight gain, and low testosterone. She runs detailed bloodwork that looks beyond a single number, so she can find what’s actually driving your symptoms and whether you can fix it without hormone replacement. Low T, an Overview

Low testosterone, also called male hypogonadism, means your body isn’t making enough testosterone. This is the hormone that fuels your energy, muscle mass, mood, and sex drive. Testosterone naturally drops about 1% per year after your late 30s, so it’s a common issue as men get older. According to the Cleveland Clinic, providers consider a level below 300 nanograms per deciliter to be low, and by age 45 roughly one in three men fall into that range.

But here’s what that number on a lab sheet doesn’t capture: the tiredness that coffee doesn’t fix. The weight that won’t come off no matter how hard you work. The sense that you’re just not firing on all cylinders the way you used to. A lot of men chalk it up to getting older and push through. You know something’s off, even if no one’s given you a straight answer.

We regularly see men who’ve been handed one of two things: a shrug, or a prescription. Maybe your numbers came back “normal” and you were sent home. Maybe you were told testosterone replacement is your only option. If either sounds familiar, you’re not stuck with just those two choices.

Low testosterone is rarely the whole story. Your energy, weight, sleep, and hormones all feed off each other, and there’s often another way to get your levels back up. For a lot of men, without hormone replacement at all.

So, What’s Actually Going On?

What’s behind the fatigue, the weight, and the low numbers? Here’s what we typically find in men:

What Low Testosterone Feels Like

For most men, it’s not dramatic. It’s low energy that doesn’t lift, weight that settles around the middle and won’t budge, and workouts that stop paying off the way they used to. Some men notice a dip in sex drive but won’t bring it up, and that’s okay. It matters, and it’s worth looking at, whether you mention it first or we get there over time.

Why Testosterone Drops

Age is part of it, but it’s rarely the whole reason. Weight gain, poor sleep, chronic stress, and too little exercise all pull testosterone down, and they feed each other. Carrying extra weight lowers testosterone, and low testosterone makes weight harder to lose. It becomes a loop. The good news is that the same loop can run in reverse once you find what’s driving it.

It’s Usually Connected to Something Else

Testosterone rarely moves on its own. Cortisol, blood sugar, and sleep all sit in the same system, so when one is off, the others often are too. That’s why we look at the full picture instead of one number. A man who’s exhausted and gaining weight might have a testosterone issue, a cortisol issue, a blood sugar issue, or all three at once.

Prostate and Urinary Symptoms

Some men come in with changes in urination or a family history of prostate problems. These aren’t testosterone issues exactly, but they’re part of men’s health, and they’re worth checking. When it makes sense, we run a PSA test to get a clear read, and we look at hormones at the same time, since there’s usually more than one thing going on.

How We Help

Once we understand what’s actually driving your symptoms, here’s what tends to help:

  • Functional Medicine Testing
Detailed bloodwork that goes past a single testosterone reading to check cortisol, blood sugar, thyroid, and the nutrient levels a standard panel skips.
  • Saliva Hormone Testing
For hormone-specific symptoms, this shows what’s actually reaching your tissues, not just what’s floating in your blood, and it reads cortisol and melatonin at the same time to catch sleep and stress issues.
  • Targeted Supplement Support
Once we see your results, we can use specific supplements to help raise testosterone naturally. For a lot of men, this works well without hormone replacement.
  • Nutrition and Lifestyle
Diet, exercise, and better sleep move testosterone on their own.
  • Chiropractic Care
Support for your nervous system so your body can shift out of constant stress mode, which affects hormones and sleep.
  • Acupuncture
Helps with stress, sleep, and the overall balance that hormones depend on.
  • PSA Testing When Needed
If prostate or urinary symptoms come up, we can run a PSA test to get a clear picture and rule things out.
  • Virtual Visits
Lab reviews, protocol check-ins, and dietary consultations can all be done remotely, no need to come in for every appointment.

Not sure which of these you need? That’s what your first appointment is for. Most men use a combination, and we’ll figure out where to start together.

What to Expect

Your First Appointment

Your first visit is a full hour. We talk through your health history, your symptoms, when they started, and what you’ve already tried. Then we figure out what testing makes sense to start with. You won’t be rushed, and you won’t be handed a prescription on your way out the door.

Testing and Results

Depending on what we’re seeing, we might run comprehensive bloodwork, add testosterone to your panel, or recommend the saliva hormone test for a fuller read. We go over your results together and explain what they mean in plain language, so you actually understand what’s going on in your body.

How Long It Takes

Be ready for honesty here: it depends. Some men feel a difference in a few weeks. Others take a few months, especially if we’re correcting long-term issues like weight, sleep, or deeper hormone imbalances. How fast things move also depends on how much of the plan you follow. We track your numbers over time and adjust as we go.

What Makes This Different

Testosterone replacement has its place, but it’s not the only answer, and it comes with trade-offs. One of them is that it can affect fertility, sometimes permanently. Before we ever go down that road, we look at whether your body can get back on track on its own. Most of the time, there’s another way, and that’s where we start.

Frequently Asked Questions

Do I have to go on testosterone replacement?
No. For a lot of men, we can raise testosterone naturally with the right supplements, nutrition, and lifestyle changes once we see what your labs show. Testosterone replacement has its place, but it comes with trade-offs, so we look at whether your body can get back on track on its own first.

How is this different from what my regular doctor offers?
Most conventional visits run 10 to 15 minutes and check one or two numbers. We take a full hour for your first visit, run detailed testing, and look at how your energy, weight, sleep, and hormones connect. We’re not just checking a box, we’re finding what’s actually driving your symptoms.

Do you take insurance?
We’re an out-of-network provider, and upon request, we’ll give you a superbill to submit for possible reimbursement. Many men find they save money compared to the copays, deductibles, and out-of-pocket costs of conventional care, especially when the tests they need aren’t covered anyway.

How long until I feel better?
It depends on what’s driving your symptoms. Some men notice a difference in a few weeks, others take a few months, especially if we’re working on weight, sleep, or deeper hormone issues. We track your numbers over time and adjust as we go.

What if I don’t want to talk about certain symptoms?
That’s okay. A lot of men aren’t comfortable bringing up things like sex drive right away, and there’s no pressure to. We start wherever you’re ready, and we build trust over time.

Do you help men outside of Elkhorn?
Yes. Patients come to us from West Omaha, Gretna, Papillion, Bennington, Ralston, La Vista, and across the Omaha metro. We also offer virtual visits, which work well for a lot of functional medicine care.
